DIRECTIONS?
Take Route 2 West from Route 128 or East from Route 495
• From Route 2 westbound, go north (right turn) on Fairhaven Road. Entrance to facility is 100 yards on right.
• From Route 2 eastbound, go north (left turn) on Sudbury Road. Turn Right on Fairhaven Road. Entrance to facility is 3 blocks ahead on
the left.
WHAT TO BRING?
Soccer cleats, shin-guards, soccer ball with name on it, water bottle, lunch, sunscreen, rain gear, footwear for lunchtime (no cleats allowed inside the building), and a change of socks is recommended.

Note: Incoming 8th and 9th Graders are eligible for a reduced rate when they register for both Slocum Soccer School programs, Day Camp & High School Prep. Be sure to email us before registering so we can create a personal discount code just for you that will take a discount off ($50) of the Day Camp registration fee at check out!
REGISTRATION REQUIREMENTS?
Completed on-line registration form, including an electronic copy of a physical examination within the past 18 months and a copy of immunization records, and payment in full will be required to participate. Pursuant to state law, no camper will be allowed to participate until all forms have been received. Registration is first come, first served. Camp has sold out in past years.
MEDICAL CARE?
A full-time athletic trainer will be on duty at all times to administer care for athletic injuries and ailments. For more serious problems, campers will be transported to Emerson Hospital.
REFUND POLICIES?
Cancellations made 3 or more weeks prior to the beginning of the session will receive a credit for the amount paid less a $50 processing fee that is good through September 1 of the following year. Credits will not be granted with less than 3 weeks notice. Regsaver Insurance (purchased through SportsEngine, as a separate transaction, during the registration process) may be purchased for a nominal fee (about $20).
MASS DEPARTMENT OF HEALTH
This camp must comply with regulations of the Massachusetts Department of Public Health and be licensed by the Concord Board of Health.

WHAT IF I MIGHT MISS SOME SESSIONS?
We understand that most players will likely miss some sessions due to other summer time obligations. In order to avoid the need to offer a prorated registration rate, the program has been priced with this in mind. And now, with our Athlete All Access page, players will be able to access the workouts to complete on their own even when they have to miss a session!
